Kyoko Sakura (prayed) Kyoko's fears are fairly standard for a girl of her age; fears being alone or seeing her friends (specifically, her fellow Puella Magi hurt.) Less typical is her fear of turning into a Witch (monsters that are born from a corrupt Puella Magi's soul, effectively,) or again, to see it happen to one of her fellow Puella Magi.
Presea Combatir (suppress) Again, there's the standard things--being injured, dying, seeing something similar happen to her friends, but more importantly Presea fears being out of control of her body. For sixteen years she was unable to act independently due to her emotions being repressed and she has a strong fear of returning to that state of emptiness.
Ellen (funnystory) Sickness. Ellen is deathly afraid of coming down with any kind of sickness, but in particular the idea of a disease that is both incurable, highly painful and/or debilitating in other ways is something that genuinely terrifies her.
Ennis (conservative) Ennis has a strong fear of a permanent death; specifically, as an immortal who (in her canon setting) can't die through normal means, she fears dying by the hand of her master or that of another immortal (in the context of Baccano! this involves another immortal placing their hand on her head and wishing to eat, at which point they would devour her body, memories and knowledge.)
